What to Prepare for Medical Treatment in Taiwan
1. Application form for arrival and departure in Taiwan for residents of Mainland China
2. Notarized Certificate of Kinship verified by the Straits Exchange Foundation (SEF)
3. Photocopies of the front and back of Mainland China's ID card, Registration Card for Resident Population, or other documents providing proof of identity
4. Passport photo taken within the past 6 months (2 inches, white background)

Please confirm if a visa is needed to enter Taiwan.
Starting from September 29, 2022, Taiwan will fully reinstate visa-free entry for nationals of countries eligible to enter the country visa-free and engage in activities such as business, exhibitions, study tours, international exchanges, family visits, sightseeing, and social visits that do not require permission. However, those who engage in activities that require permission according to the laws and regulations of domestic agencies must still apply for permission from the government and apply for special entry permits (visas) from the Office of the Ministry of Foreign Affairs.

For those who do not meet visa-free regulation standards, please find instructions and relevant documents to apply for a special entry permit (visa) with the foreign missions of the Ministry of Foreign Affairs.

2. Non-Taiwanese nationals: passport and ID card
B. Original Marriage Certificate (for IVF, artificial insemination, embryo transfer, etc.)
1. Marriage Certificate from your country/region
2. Household Registration Book/Household Registration Transcript (for foreign spouses without a National ID card)
